NLCL would like to collect pre OGP service fees for the following categories:
Infant
Toddler
Preschool
After School
Before and After School
*You can include fees that you were collecting for either the year before you joined OGP or for the same year you joined. The fee cannot be the rate that you were receiving from OGP.
Example: If you joined OGP in May of 2018 and in April of 2018 your preschool fee was $42.00 then you would enter $42.00 for Preschool and the Pre OGP-Year would be 2018.
If you did not provide any of the services listed then please leave that category blank.
The collection of this data is anonymous and your centre will not be identified as having contributed to this collection.
